Medulla drops some solid level 125+ cabal gear - particularly hats and boots - that players have farmed for. Fortunately, many drop in other in other places that are much easier to farm as well. Here's a look at the level 125+ cabal gear sets and where to farm for them!
Medulla & Co.
Medulla is one of those bosses that you probably want to avoid if you cant. He's located in The Headquarters (that's "head quarters" literally - the brain of a giant squid) of Sepidious and is the final boss of Empyrea Part 1. Eric Stormbringer has some great tips and strategy recommendations for Medulla here.
Medulla drops most of the gear, and is the only boss to drop the robes (with the exception of the universal robe - the universal set is dropped more frequently from other bosses). The robes are sometimes used for stats (but don't live up to most of the serious end-game robes), and are also used for stitching. Lucky for you, the most commonly used items - the hats and boots - are dropped by various bosses around Empyrea Part 1. The most common place to farm for the hats and boots is the Secret Tunnel with the enemies Gannon, Madd'n, and D'viss. The decks drop from the Beastman Fight Club bosses.

Why These Items?
There are also some other Cabal items, including wands (banners) dropped by Medulla and athames, rings, and amulets. However, these items not only have poor stats but also aren't used hardly at all. Anything which is an exclusive drop from Medulla or an ultra-difficult boss isn't going to be super common, so when you stack bad stats on top of that, you just don't see them. So they're not covered here. as I'll mention, the pieces here range from semi-common to super common in setups now.
Why They Matter
Over the course of this month, I'll be doing a sort of "Significant Gear Series" that covers the most important sets and items that have defined where wizards farm, how they build their setups, and more. This will be an opportunity to compare stats and discuss structure. We'll cover everything from Waterworks to Darkmoor to some of the more recent sets. In the future, we may go further back.
When I talk about the trouble with end-world bosses, Medulla is one of those bosses that I'm talking about. It's fortunate, then, that a lot of the gear is dropped elsewhere. I cannot comment on the difficulty of the Beastman Fight Club, as I have yet to attempt that dungeon (and also... fight club rules, duh). But that's only the decks. In fact, if you spend much time farming the Secret Tunnel enemies, you'll know how incredibly easy they are. This distribution of gear drops makes Medulla a little less intimidating.
The robes are somewhat rare, and are not often used for stats given the increasing availability of good robe options. The hats and boots, however, are everywhere. They're by far the easiest of so many max-level items to get. They are focused heavily on offense, both giving critical chance and significant damage. The hat, like other more recent pieces, gives up the resistance that Darkmoor items have.
